No Dentist-Patient Relationship
Using this website does not create a dentist-patient relationship between your child and Children’s Dentistry or any of our providers. A dentist-patient relationship is established only after we have seen your child in our Santa Rosa office, reviewed your child’s health history, and agreed to provide care.
In an Emergency, Call 911
If your child is experiencing a dental, medical, or any other emergency, do not use this website’s contact form. Call 911 or go to the nearest emergency room immediately. We cannot monitor the website contact form in real time and cannot respond to emergency situations through this channel.
Appointment Requests
Submitting an appointment request form does not confirm or guarantee an appointment. After we receive your request, a member of our team will contact you to discuss availability and confirm a date and time. An appointment is confirmed only when we have both agreed on a specific time.
Parental Consent for Treatment
California law generally requires the consent of a parent or legal guardian before a child receives dental care. The website cannot be used to provide that consent. Consent is obtained through forms and conversations at your child’s visit.
